Getting Started

The Initial Consultation
I need to fully understand each particular situation before reaching a proper diagnosis and offering you my recommendations. Therefore, considerable time and effort is devoted to the initial evaluation. An adult or an older adolescent will usually come alone to the first appointment. In the case of a younger child, the parents may be seen first and the child at a later date, or everyone may be asked to come in together for a longer session. At the end of the evaluation, I will offer my impressions and recommendations.

ONE: Confirmation E-Mail
After the first phone conversation, you will receive a confirmation email with the date and time of the initial consultation. Included within this email will also be the BUILDING ENTRY CODE. Be sure to make note of this code, so that you can let yourself into the office and relax in the reception area.

TWO: New Patient Packet
Before your first appointment, please download and carefully read either the NEW ADULT PATIENT PACKET or the NEW CHILD PATIENT PACKET from the “FORMS” section of this page. In an effort to use the first session in the best way possible, please completely fill out each form and either email them to me as an attachment, fax them to the office, or bring the forms with you to the first session. Otherwise, they will need to be completed during the initial evaluation time.

As you explore this website, please look at the “RESOURCES” page on which there are several questionnaires related to potential issues we might discuss. You are welcome to view, download and fill-out the ones that seem applicable to you and bring these with you to the Initial Consultation as well.

FOUR: Additional Information
Additionally, please bring copies of any relevant testing, school reports, or medical records you believe may supplement your patient file at Oceanside Mental Health.
